Performs LTC Facility site visits independently or as a member of a team Responds to Facility needs within 24 hrs. of an outbreak as part of a Rapid Response Team (MD,RN,SW) Maintains knowledge/relationships with community health care providers and resources to support Resident transitions as appropriate Identifying and providing guidance to facilities regarding resident/staff behavioral health needs/resources Conduct trainings for facility staff so they are better equipped to recognize behavioral health needs of residents and staff Provide crisis intervention support including emotional support to staff   Position Requirements Knowledge: Resources within Philadelphia and surrounding counties health systems and LTC Facility communities. Understanding of patient social, psychological and/or environmental needs related to the impact of long-term care living situation. Intimate knowledge of the American health care system. Understanding of public and private health insurance and payment mechanisms for long term care. Palliative care/bereavement counseling a plus. Skills Required: Strong interpersonal skills and team-based care skills, Crisis intervention and management skills. Psychosocial and grief counseling skills. Organization and attention to detail. Written and verbal communication. Time management; great sense of urgency. Self-manages; great follow-up/follow-through. Strong client assessment skills. MSW required. Minimum of 1-3 years of medical social work experience required in the settings of inpatient hospital, ambulatory care, palliative care, or long-term care.   This position is currently not accepting applications.
