Description

Social Worker: Site Visit and Rapid Response Team needed for a full-time, temporary (5 month) position in the Philadelphia area.

  • Performs LTC Facility site visits independently or as a member of a team
  • Responds to Facility needs within 24 hrs. of an outbreak as part of a Rapid Response Team (MD,RN,SW)
  • Maintains knowledge/relationships with community health care providers and resources to support Resident transitions as appropriate
  • Identifying and providing guidance to facilities regarding resident/staff behavioral health needs/resources
  • Conduct trainings for facility staff so they are better equipped to recognize behavioral health needs of residents and staff
  • Provide crisis intervention support including emotional support to staff
 
Position Requirements

Knowledge: Resources within Philadelphia and surrounding counties health systems and LTC Facility communities. Understanding of patient social, psychological and/or environmental needs related to the impact of long-term care living situation. Intimate knowledge of the American health care system. Understanding of public and private health insurance and payment mechanisms for long term care. Palliative care/bereavement counseling a plus.

Skills Required: Strong interpersonal skills and team-based care skills, Crisis intervention and management skills. Psychosocial and grief counseling skills. Organization and attention to detail. Written and verbal communication. Time management; great sense of urgency. Self-manages; great follow-up/follow-through. Strong client assessment skills.

MSW required. Minimum of 1-3 years of medical social work experience required in the settings of inpatient hospital, ambulatory care, palliative care, or long-term care.
