Seeking a Passionate Community Health Worker

30-40 Hours per Week

Our Community Health Worker (CHW) implements program imperatives for identified patients to promote optimal health and wellness.  A CHW is a public health worker who has a heightened understanding of the community served through their specific program. This understanding enables the CHW to serve as a liaison, link and educator between services and the community to promote services and ultimately enhance community health. A CHW builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, data collection, social support and advocacy. Trained as a Financial Coach, this position will work with HOPES Patients to help them achieve financial stability. Through individual and group activities, this CHW will educate and empower patients in budgeting, saving, and achieving financial wellness. Our CHW engages in patient identification and outreach, program implementation and patient education.

Position Requirements
  1. Bilingual required.
  2. Prior experience working with target population required.
  3. Proven ability to maintain professional conduct, including appropriate boundaries required.
  4. Able to multitask, develop organizational structures and set goals required.
  5. Able to work autonomously as well as collaboratively with all team members required.
  6. Able to exercise empathy and understanding required.
  7. Able to communicate and listen effectively required.
  8. Prior experience in financial education preferred.
  9. Completion of the CHW certification program preferred. Should be completed within 90 days of employment.

We are seeking an individual who demonstrates ethical conduct and a respectful attitude when interacting with patients. Our CHW establishes appropriate boundaries understands the needs of participants while exercising sound judgement. CHWs at HOPES advocate for the needs of patients and the community served. They have a strong patient focus, excellent communication skills and embrace teamwork. With a natural inclination for teaching and educating, the CHW empowers patients to achieve optimal health. Our CHWs always exercise cultural respect and humility to make a connection, watch for cues and seek opportunities to learn about and celebrate diversity.

 
About the Organization Northern Nevada HOPES is a nonprofit community health center in downtown Reno, NV that offers integrated medical care and wellness services. We welcome patients, wherever they are in life, and provide them with compassionate, high-quality healthcare.

Our growing list of services includes adult and pediatric primary care, chronic disease management, wellness and nutrition, behavioral health counseling, substance use counseling, case management, a pharmacy, and more. Our team-based approach to care gives patients access to a range of comprehensive services in one convenient location.

For medically underserved populations, our one-stop-shop healthcare model reduces barriers to care and increases the likelihood of maintaining long-term health. HOPES accepts most insurance plans, Medicaid, Medicare, and the uninsured. For the uninsured, we offer discounted services based on a sliding fee scale.
