SAFY of South Carolina is seeking a full-time Intake Coordinator responsible for the intake referrals and placement of youth into foster homes.

As the Intake Coordinator you would:

  • Maintain intake and placements to assigned divisions.
  • Gather information supplied by referral agencies and enter referral information in SOAR.
  • Review and assess initial intake information from a therapeutic perspective to determine course of action and potential match to division and foster parent; verify licensing status; contact referral source and discuss potential matches.
  • Notify appropriate Treatment Director about referral; communicate referral information to recommended foster parents and clarify clinical information.
  • Serve as a liaison between referring agencies, SAFY divisional offices; specialty directors and foster parents; arrange for TFP to meet youth and accompany TFP to initial visit, school meetings and court proceedings if needed. Once placement is accepted, complete seven-day treatment plan/referral report within 7 days of placement.
  • Staff cases with Treatment Director and Case Coordinator. Compliance/Reporting:
  • Document referral outcomes in SOAR
  • Request required documents from referral source via emailing checklist. Email/distribute all information received related to referral/placement to Treatment Director and Case Coordinator. • Maintain weekly Roll Call schedule with Treatment Director or designee to ensure up to date open bed list. • Maintain awareness of contract issues with different agencies; communicate with SAFY Finance Department any relevant information regarding placements; stay abreast of Interstate compacts in regards of placements.
  • Attend referral meetings with Department of Child Services, Probation or other agencies as necessary.
  • Assist with youth transfers and respite arrangements.
  • Assist with file audits and recertification.
  • Remain abreast of rules and regulations regarding foster care facilities and/or placements and Diagnostic and Statistical Manual of Mental Disorders in relation to clients' therapeutic needs. Attend all required staff meetings. Submit all required reports within specific time frames; meet monthly with supervisor and participate in assigned committees.
  • Quarterly attend Divisional Foster Parent Activities to Connect and network with Parents
Position Requirements Bachelor Degree with LPC/LSW required, Masters preferred. Negotiation skills and clinical knowledge of matching process a must.
