What are we looking for in a Case Manager?
In this position, Case Managers provide stabilization, case management, and supportive counseling to clients who are referred by law enforcement to the Behavioral Health Diversion program. Case Managers on the Diversion team work closely with Crisis Outreach Professionals to provide services and education to clients and their family members, and to maintain communication with the referral sources. The Case Manager will coordinate the client's intake assessments and ensure they are enrolled in services with the appropriate team, and then assist with keeping the client engaged with treatment. To be successful in this role, Case Managers must enjoy variety in their day, possess exceptional communication skills, and be able to solve problems creatively. The ability to work effectively with multiple internal and external partners is essential.
Case Manager duties may include:
Education & Experience: Bachelor's degree in social work, psychology, social services or related field.
Professional Credentials: Eligible for Agency Affiliated Counselor registration
Other Requirements: Driver's License, Vehicle Liability Insurance
