JOB SUMMARY: Responsible for establishing relationships in the community to aid in the linkages to MAT treatment



  • Meet with individuals at time of engagement and assesses their needs
  • Discusses treatment options with patients and completes referral to treatment
  • Coordinates with treatment agencies for immediate service linkage via Catholic Charities Transportation Services and/or to determine pickup time for patients by other identified providers that will aid the individual in their recovery process.
  • Verify insurance coverage utilizing eMEVS. Completes Presumptive Eligibility screening as needed
  • Connects patients to community resources such as: food pantry, medical services, emergency housing services, etc. utilizing NowPow
  • Establish and maintain good working relationships with service providers
  • Develop rehabilitative and discharge methods unique to population and program goals.
  • Maintain accurate progress notes and data identifying needs, strengths and gaps in services.
  • Follow and verify engagement in and satisfaction with identified services and provide support as needed.

Promote and engage healthy family relationships.

Position Requirements


High School / GED + 5 years of human services experience; OR relevant professional credential such as CPRP, CRC, CADC + 2 years related exp., OR a combination of one or more years of college + related work or life experience together equaling four years; OR Bachelor’s degree in human services field, rehabilitation, nursing, or a related discipline.
