What are we looking for in a Care Coordinator?
Care Coordinators are part of the WISe (Wraparound with Intensive Services) program, which serves youth who have mental health diagnoses and are experiencing severe behavioral disruptions that impact their functioning in family, school, and social environments. WISe is an evidence-based model with a collaborative, cross-system approach to creative problem-solving. Care Coordinators work with youth and their families to develop and implement individualized, cross-system care plans that work toward the family’s long-term goals. Services are provided where and when convenient for the family, sometimes out in the community and outside of regular business hours. Care Coordinators must be skilled in managing team conflict, engaging a variety of personalities, facilitating meetings, and communicating with various audiences. Travel throughout Klickitat County and a flexible schedule are expected.
Care Coordinator duties may include:
Education & Experience: Bachelor's degree in psychology, social work, social services or related field. Prior experience in a related role is preferred.
Professional Credentials: Eligible for Washington State Agency Affiliated Counselor Registration
Other Requirements: Driver's License, Vehicle Liability Insurance
