About the Organization CP Rochester and Happiness House are private nonprofit health, educational, residential and human service organizations that have been serving communities in the Finger Lakes Region since 1969 and the Greater Rochester Area since 1946. Today with sites in Rochester, Canandaigua, Geneva, Gorham, Midlakes and Waterloo, our agencies offer an array of educational, therapeutic, recreational, residential and family support programs and services for children and adults with and without disabilities. On average, our agencies serve 3,650 families each year and employ approximately 600 people.


Our staff consists of dedicated and knowledgeable professionals and clinicians with expertise in the areas of Special Education, Early Childhood Education, Physical, Occupational, Speech and Music Therapies, Psychology, Service Coordination, Nursing, Dental Services, and Developmental Disabilities.


Committed to serving individuals with autism, traumatic brain injury, neurological impairments, as well as a wide variety of developmental disabilities such as cerebral palsy and Down syndrome, our staff partners with individuals and their families to support them in meeting their goals.

Description

Full Time (35 hours per week) position at our Canandaigua site.

Required Education and Experience:

  1. Education and Experience Requirements:
  • Masters Social Work, Masters Psychology, Registered Physical Therapist, Registered Nurse, Certified Special Education Teacher, Speech Language Pathologist, Registered Occupational Therapist with one year experience in providing service coordination/case management to individuals who have a disability. Or
  • Bachelor's Degree in health and human services or a related field with three years experience in service coordination/case management.

2. Knowledge of community resources, social service agencies and means of accessing services.

3. Valid NYS driver's license.

Responsibilities include, but are not limited to:

  1. Imparts agency philosophy of 'Equal Opportunity, Independence and Realization of Individual Potential' to children, families, program staff and community resources.
  2. Maintains confidentiality of information relating to individuals and their families in accordance with agency policy.
  3. Identifies the needs of disabled individuals and their families.
  4. Provides information, referral and advocacy services to individuals of all ages with disabilities and their families.
  5. Works with families/individuals to develop appropriate plans for the delivery of services.
  6. Coordinates implementation of the services by assuring appropriate communication and cooperation is maintained by all supports and providers of services outlined in the plan.
  7. Establishes and maintains a cooperative working relationship with area human service agencies, local school districts and individuals, as pertinent.
  8. Arranges for clinical assessments to determine the person's functional status, service needs and recommendations for interventions.
  9. Assists the individual and/or family to gain a variety of skills including self-advocacy, problem solving and skills to increase their independence.
  10. Assures needed assessments and documentations are performed, as required by DOH/TBI regulations.
  11. Participates in staffing, case conferences and other client-related meetings upon request.
  12. Coordinates and develops components of the individualized service plan.
  13. Maintains, at a minimum, face-to-face contact requirement with the recipient and, if appropriate, his/her family or advocate, per applicable regulation.
 
Position Requirements

A.Minimum Qualifications

  1. High School Diploma.
  2. One year of experience providing services to individuals with disabilities, preferably working with individuals with traumatic brain injury.
  3. Knowledge of community resources and means of accessing these resources.
  4. Valid NYS driver’s license.
  5. Demonstrates basic reasoning and writing skills.
