Summary/Objective
This position will provide direct services — including Day Habilitation and Non-Medical Transportation Services — for adults with developmental disabilities. Essential Functions
Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
- As assigned, assist in the general referral, admission and transition of persons served into and within the organization via participation in staff meetings
- Attend ISP meetings as assigned, and implement all sections of the ISP
- Check and record medical needs of consumers — including nursing delegation and any self-medication programs
- Provide transportation to consumers, as needed or assigned
- Assure that proper procedures are followed in regard to consumers and their inherent rights
- Produce daily progress notes for each consumer and communicate important information by all appropriate means
- Assist in the periodic review and update of consumer files
- Assist as needed with ambulation and/or transfer of consumers with mobility impairments
- Attend staff meetings, and other meetings, as assigned
- Maintain safe and healthy conditions in the building and vehicles and report maintenance needs to the Program Manager promptly
- As assigned by the Program Manager, complete all required documentation related to funding, employment or general operations including but not limited to the following: Time reporting, Service delivery, Skill development, Unusual and major unusual incident reporting, and Other incident reports as needed
- Assist in the timely submission of all records, files, financial records, etc. pertaining to specified individuals
- Perform all other duties as assigned

Required Education and Experience
- High School Diploma or equivalent.
- Two years’ experience working with people with disabilities.
- Valid Driver's License and Insurable Driving Record.
Preferred Education and Experience
- Five years’ experience working with people with intellectual and developmental disabilities.
Additional Eligibility Qualifications
- Must be able to function independently and have flexibility, personal integrity and the ability to work effectively with consumers and other staff
- Must be able to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile people within the facility
- Must possess the ability to make independent decisions when circumstances warrant such action.
