Description

Key Responsibilities

  • Partner with clients and families to formulate and implement individualized client-centered goals with focused, targeted and measurable outcomes.

  • Provide client care in collaboration with the client/family to plan, implement and evaluate care, documenting the habilitative/rehabilitative process according to hospital policies.

  • Interface and problem solve with members of the interprofessional care team and community providers as appropriate to ensure an integrated approach to care.

  • Provide assessment, direct service, consultation, mediation and education to client, family and community providers with focus on maximizing a client’s function in the areas of self-care, productivity (school and play skills) and leisure (community participation) to clients with progressive neuromuscular disorders (eg. DMD, SMA)

  • Collaborate with the client/family to plan, implement and evaluate care

  • Engage in evidence-based practice including but not limited to using outcomes measures

  • Participate in Occupational Therapy and Program related research where applicable

  • Document in accordance with professional standards of practice and Holland Bloorview policies

  • Contribute to the development of professional practice through participation and leadership in program, organizational and professional activities

  • Ensure practice issues are addressed using appropriate lines of communication within the organization

  • Participate in quality improvement initiatives,support the maintenance of a safe and healthy work environment and advance a culture of client/patient safety through work and daily practices

  • Adheres to professional standards of practice as established by the College of Occupational Therapists of Ontario, and the policies/procedures at Holland Bloorview

  • Other duties as assigned

Qualifications

  • Master’s degree in Occupational Therapy (or Bachelor degree in Occupational Therapy plus other Masters)

  • Paediatric experience with clients with physical disabilities preferred

  • Experience with clients with progressive neuromuscular disorders (eg. DMD, SMA) preferred

  • Current ADP Registered Authorizer Number for mobility devices required

  • Experience in assessment for seating, home adaptations, vehicle modification and other equipment required

  • Current BCLS Certificate – Basic Rescuer (C)

  • Registered with the College of Occupational Therapists of Ontario

  • Current Membership in the Canadian Association of Occupational Therapists is an asset

  • Specialized clinical education an asset (e.g. NDT training)

  • Demonstrated ability to communicate effectively with clients, families, vendors and members of an interdisciplinary team

  • Demonstrated ability to organize time and meet deadlines

  • Demonstrated ability to work independently and effectively and efficiently manage fluctuating clinical responsibilities
