JOB DESCRIPTION

Patient Referred Care Coordinator

OVERVIEW: The primary responsibly of this position will be processing PRC Medical Travel for our PRC eligible population and any persons in between. This position will also be working closely with our referral coordinator to coordinate travel arrangements for paratransit eligible patients (Elwha Patients only). Processing will include following policies and procedures set forth by both IHS and intertribal policies and procedures.

POSITION STATUS: Full Time, Benefits SALARY LEVEL: DOQ

WORK SCHEDULE: Monday - Friday, 8:00am to 5:00 pm

RESPONSIBILITIES:

The duties of the Patient Referred Care Payable as follows:

Back up general operations of the Patient Referred Care

  • Will be responsible for paying the bills for outside patient care for those patients who are PRC eligible,
  • Will primarily be responsible for medical transportation coordination for patients, and hotel requests and work with the Referral Coordinator to make sure when appointments are made for the patient the Purchase Order is made, hotel and transportation is arranged.
  • Confirms the identification of patients who are or may be eligible for alternate resources and assist the Benefits Coordinator in helping the PRC eligible to apply for Alternate Resources.
  • Advocating on behalf of patients with all alternate resources activities.
  • Effectively educating patients on alternate resources.
  • Must maintain confidentiality and confidential records for all patients.
  • Must be a team player working with all staff in a professional manner.
  • Will provide monthly reports for department measures and qualitative work progress.
  • Take and review application for health care services (PAO-21) for completeness, collects or requests missing or needed information to determine patient's eligibility for Patient Referred Care(PRC), maintains PRC Commitment Registers, maintains PRC individual and PRC general files.

DESIRED QUALIFICATIONS:

  • 2-3 years of work experience in a medical/professional office environment with excellent customer service skills
  • Knowledge of Health Insurance resources.
  • Strong computer skill; RPMS is preferred
  • Must possess strong customer service skills/interpersonal interactions; ability to handle difficult situations and encounters constructively.
  • Ability to follow directions and absorb quantities of information and materials necessary for the efficient performance of the assigned tasks and duties.
  • Must be self-motivated.
  • Must have the ability to comprehend and apply Patient Referred Care Rules and Regulations practices to enhance work proficiency.
  • Valid Washington State Driver's License.
  • Minimum of a High School Diploma or GED.
  • Knowledge of medical terminology;
  • Knowledge of ICD-10 coding.
  • PHYSICAL DEMANDS:
  • Requires standing, sitting, stooping and bending intermittently
  • Requires ability to lift up to 25 lbs. infrequently
  • Requires ability to see/understand colors, alpha and numerical characters, and have visual acuity
  • Requires ability to hear and understand verbal instructions
  • Requires use of standard office equipment such as computers, phones, faxes, and photocopiers

WORKING CONDITIONS:

  • Work is performed in a medical office environment
  • Involves interpersonal contact with staff, patients and agencies
