We are seeking a mission focused professional to serve as a PRN Case Coordinator responsible for the case management functions of a caseload of foster youth and respective foster homes in Fort Wayne, Indiana!

As a PRN Case Coordinator, you will:

  • Conduct school visits to sit-in on Individualized Education Plan (IEP) meetings. Assure that health care needs of youth are being realized including medical, dental, vision and hearing as well as special needs. Complete assessments as required by state rules, agency policies, and/or contract requirements.
  • Prepare Individualized Treatment Plans, DAF’s, and Mental Health Service Plans according to established time frames involving all appropriate parties as required by policy and regulations of the state.
  • Timely document outcome of home visits, support sessions, interaction with parties involved in the youth’s care and incidents as required by policy, regulations and contract requirements.
  • Will complete a minimum of weekly home visits to assure that all children are safe
  • Agrees to meet all evaluation and reporting requirements that may include monthly updates, quarterly reports, and court reports as requested by the Department of Child Services.
  • 90% of families with have face-to-face contact with service provider within 3 days of receipt of referral
  • On call will be required on occasion.
  • Provide support to foster parents and foster children. Advocate for and attend Child and Family Team meetings. Attend Court in regards to the foster children served.
Position Requirements

What you will bring to this role:

  • Bachelor's or master’s degree in the field of social work, counseling, psychology, sociology, or child & family studies.
  • Relevant experience preferred, with experience in child welfare and adoption being highly preferred
