Share Email Opening Title Counselor - (CSS-Community Support Services)  Description Responsibilities/Qualifications Provides all needed services to consumers based upon the level of functioning and disability and the consumers’ needs. Assures departmental goals or objectives are accomplished within prescribed time frame by performing the job responsibilities personally. Maintains confidentiality and immediately reports to management breaches in confidentiality Develops, implements, monitors and updates the individualized rehabilitation plan Offers on-site services and support appropriate to meet the consumers’ needs Assesses and/or evaluates consumers and families at intake and/or admissions, during ongoing service delivery and at discharge; collaborates with treatment providers to establish comprehensive assessments and service plans Performs the following duties as a member of an interdisciplinary team (internal and external) - conducts outreaches, assesses consumers and articulates (verbally and in written documentation) the integration of consumer history and assessment information; mentors new staff Formulates service plans containing relevant goals, behaviorally measurable objectives (that serve as indicators for evaluating progress) and realistic time frames for achievement Records/documents the following information in the consumer’s clinical record; assessments, service plans, progress notes (DAP format) and discharge summaries, with all entries authenticated and dated Safely operates and utilizes Agency vehicles while transporting clients to and from specialized medical appointment; follows all policies and procedures related to vehicle safety and maintenance as outlined in the Agency’s Transportation Manual Contribute to the development, implementation, monitoring, and update of the individualized rehabilitation plan, perform rehabilitative skill development     Position Requirements Education/Experience A bachelor’s degree in one of the helping professions (e.g. social work, human services, counseling, psychiatric rehabilitation, psychology, or criminal justice), OR Bachelor’s degree in a field other than a helping profession AND 2 year experience in a community behavioral healthcare setting Note: Certification as a psychiatric rehabilitation practitioner may be substituted for one year of experience above A master’s degree in social work, rehabilitation counseling, psychology, counseling, or other related behavioral health or counseling program     Full-Time/Part-Time Full-Time   Shift Days   Location Main Site   About the Organization CarePlus New Jersey is a northern New Jersey provider of recovery-focused mental health, substance abuse, and family services that address the unique needs of individuals with a holistic approach to recovery and overall wellness.
