Job Summary: Assumes responsibility for implementing the Farmworker Community and Family health Program for migrant and seasonal Farmworkers operated by Clinica de Salud del Valle de Salinas. Works under the supervision of the Outreach Manager and other Management personnel as designated.
DUTIES AND RESPONSIBILITIES:
Demonstrates Competency in the Following Areas:
Attend inter-agency and/or community meetings to become familiar with the social services available. Disseminate this information to farm workers.
Develop or update a simple survey in English/Spanish (or other language as appropriate) to assess the basic social and medical farm worker needs.
Provide referrals (i.e. medical care, medicine, social services-) and follow-up.
Prepare and conduct culturally and linguistically appropriate health education presentations on hypertension, diabetes, tuberculosis, pesticide exposure, and other topics identified by farm workers.
Facilitating weekly training sessions on the content of Farmworker Community and Family health Program and on Principles of health promotion.
Document farm worker encountered statistics, using appropriate forms and monitoring data collection for the evaluation of the program.
Provides verbal and written feedback to supervisors regarding progress of the program.
Visit with farm workers where they live and gather to share health information.
Counsels and inform the patient population about the medical and dental services available to them through CSVS.
Continually remains informed of changes to programs and eligibility requirements to include but not limited to SOFP, CHDP, VFC, BCEDP, EAPC, Medi-Cal, MediCare, etc.
Assists in scheduling of patients for services at clinics throughout the county.
Brings patient concerns to the attention of the administration and providers.
Assists in development of communication links and networking between Promotoras, health Aides and CSVS and agency personnel.
Maintains a good working relationship within the department and with other departments.
Ensures documentation meets current standards and policies.
Treats all patients, family and fellow staff members with respect and dignity.
Maintains established office policies and procedures, objectives, performance improvement program, safety, environmental and infection control standards.
Answers telephone calls in a professional manner, directs calls appropriately.
Maintains a good working relationship with all staff members of the office, working as a team and performing teamwork duties as needed.
Manages and operates equipment safely and correctly. Performs preventative maintenance.
Demonstrates the ability to be flexible, organized and function under stressful situations.
Should be familiar with primary health care delivery, social service, or counseling system
Ability to organize information, and communicate ideas in written and verbal form
Knowledge and experience in health education program development
Experience in direct service to migrant and seasonal Farmworkers
Must be committed to empowerment through education of minority and disadvantaged groups in our society
Performs other duties as assigned.
