We are seeking mission focused professionals to serve as a part time Case Coordinator responsible for providing in-home support services for the Bowling Green Division's foster parents and children.

As a Case Coordinator you would:

  • Create your own schedule based on your caseload and availability of your assigned families. This includes potentially working until 8pm several nights a month, based on the needs of the families.
  • Provide in-home, direct services to families and children in your caseload. Generally, caseloads consist of 7-9 clients with whom you visit and support 2-3 times a month.
  • Potentially be asked to assist in our rotating on-call schedule, as needed.
  • Create documentation for all services you provide in our Electronic Record System. Documentation includes Assessments, Diagnosis, Treatment Plans and Progress Notes, and Monthly reports. Overall documentation time is about 20%.
  • Develop and maintain consultative and liaison relationships with DCBS and other members of the child's treatment team (biological parents, school and SAFY team members)
  • Travel to families near your home, in and around Bowling Green and surrounding counties. Overall hours being worked outside of the office are estimated to be around 60%, which includes traveling to and from visits and time spend at the home conducting a home visit.
 
Position Requirements

Education: 

  • Bachelor’s degree in the field of social work, counseling, psychology, sociology, child & family studies, or other approved related field of human services.

Experience: 

  • Two years of relevant experience is required with above listed degrees, experience in child welfare preferred, OR relevant experience in working with children in general. Five years of relevant experience required with related field of human service degrees. 
  • License in state is preferred, not required 

Skills:

  • Strong organizational skills to prioritize workload
  • Excellent communication skills (oral and written)
  • Demonstrated skill of writing treatment plans, case notes, etc.
  • Operate a computer in regards to Microsoft Office and agency database system
