JOB DESCRIPTION
TITLE: SPEECH LANGUAGE PATHOLOGIST
REPORTS TO: THERAPY DIRECTOR/SUPERVISOR/COORDINATOR
JOB SUMMARY
To identify, assess, and provide treatment for communication and swallowing function and their disorders; complete required documentation and also participate in program planning and implementation, discharge planning
QUALIFICATIONS
Master's degree in communication disorders
Must meet the requirements for State of Illinois Department of Professional Regulation
Must possess the Certificate of Clinical Competence (CCC) of the American Speech and Hearing Association (ASHA) or actively completing the certification in clinical fellowship year (CFY)
Must speak, write, read and comprehend the English language
Must possess current CPR certification and maintain yearly
Must possess OSHA certification and maintain yearly
Must have proof of current physical exam and maintain yearly
Must have results of TB Test and Hepatitis Vaccination or waiver
Must have the ability to work independently and make decisions when circumstances warrant such actions
Must have the ability to effectively communicate with patients, family, staff, visitors and physicians
JOB DUTIES/RESPONSIBILITIES
Functions according to the ASHA Code of Ethics
Identifies potential patients in need of speech-language pathology services through screening protocols
Evaluation of deficits, strengths, and needs required for communication and swallowing
Develops intervention plans; sets goals and determines patient's progress potential based on patient's ability to reach full potential
Adapts activities, environment, tools, materials according to the needs of the patient
Modifies patient's response to intervention and modifies treatment plan as needed
Provides counseling to the patient, family, caregivers and staff regarding communication/swallowing status, needs and emotional adjustment to deficits
Screens auditory function and makes appropriate referrals
Terminates services when maximum potential is reached and formulates discharge plan and recommendations
Follows policies and procedures required in setting
Attends facility departmental meetings
Initiates and continues staff in-services
Fulfills documentation requirements in a complete, appropriate, and timely manner
Maintains records required by practice setting, third party payer
Schedules and prioritizes own workload
Assists in establishing or upgrading programs; seizes opportunities to take on new programs and assignments
Establishes short and long term goals for self and follows through in a timely manner
Assists in maintenance of quality assurance
Communicates & collaborates with team members on patient's status
Supervises SLP interns
Attends appropriate care plan meetings when requested
Assists in maintenance of ST treatment area
SUPERVISION
Speech pathologist currently in CFY will be supervised according to ASHA's guidelines for certification
Evaluations and treatment plans written by the CFY must be co-signed by a licensed and certified SLP
