TRC Clinical Case Manager-LSW/LAC (Per Diem)

Description of duties and responsibilities

The Clinical Case Managers will act as a “first responder,” addressing complex crises involving both psychological and environmental components. They will be the single point of contact for victims and will provide most services, including clinical case management, psychotherapy, advocacy, substance abuse treatment, and assertive outreach as needed. They will work holistically, ensuring that care and discharge plans meet the physical, social, and emotional needs of patients. Responsibilities include but are not limited to:

Coordinates all of the client resources needed

Completes a comprehensive assessment of the clients’ needs and identify resources, including basic needs and assist in accessing these services

Actively engages with the client, including going with client to court dates or to medical appointments and housing and financial assistance organizations

Provides therapeutic and trauma informed case management using a wraparound approach

Provides therapy sessions using EBP models to increase client’s coping strategies, emotional wellbeing and to increase support systems and protective factors as well as improve functioning

Responsible for documenting all aspects of the services provided as well as creating a care plan with client. (psychological and practical need)

Links clients to community resources as needed (housing, food, health insurance and medical care, victim compensation)

Helps client address substance abuse issues as needed in treatment and referrals if needed to detox or residential treatment

Provides telephonic crisis intervention to clients on a rotating basis

 Part of a multidisciplinary team

Education/Qualifications 

Master’s degree with professional license in the mental health/social work field.

(LAC, LSW)

Full time Clinical Case Managers must have at minimum a provisional clinical license; advanced clinical license preferred

CADC or substance abuse knowledge and/or  treatment with multicultural clients (LGBTQ, underserved minorities) prefered

Reports directly to the Bergen County TRC Program Coordinator

Knowledge of trauma-informed care and evidence-based practices, such as Trauma Focused Cognitive Behavioral Therapy and/or EMDR/ARC/Seeking Safety

Experience with underserved populations

Strong clinical intervention skills

Strong communication, listening, organization and problem solving skills

Travel is required between service area

A valid driver’s license and must be insurable by agency’s automobile insurance
