Program Aide - Healthy Families (Hudson) Full Time

Key Responsibilities

Provides in-home education and supportive services to new and expectant parents in Bergen and Hudson Counties, especially those families overburden by stressors placing them at risk of child abuse and neglect. Assures departmental goals and/or objectives are accomplished within prescribed time frame by performing the job responsibilities personally.

 

  • Conducts on-site or off-site assessments via face-to-face interaction

  • Part-time staff completes between 11-15 screens per month and between 8-10 assessments per month

  • Tracks all referrals, screens, and assessments in the HF-TIP database system.

  • Interviews and conducts the identified assessment tool with parents to determine eligibility for home visitation services

  • Promotes the program in the community

  • Conducts intensive home visitation to assure parenting skills, child development, child health, and other aspects of family functioning/needs are being met

  • Primary contact with family is to occur within the family’s home with each visit lasting for at least one (1) hour

  • Maintains a regular and long-term professional relationship with the family

  • Interventions are to be family-centered and strength-based

  • Responds to the individual needs of their clients and families

  • Is culturally sensitive and appropriate when working with culturally diverse community

  • Relates to families who may not share basic commonality, including values system and behavior norms

  • Full-time staff maintains a case weight of 30 and will serve between 15 and 25 families

  • Tracks all assessed families not enrolled into the program

  • Assists the family in establishing goals and plan for the accomplishment of those goals

  • Recognizes and identifies gaps in services and actively collaborates with the community and other providers to develop resolutions

  • Identifies and refers families to other supportive agencies, including healthcare appointments

  • Makes referrals to appropriate community-based agencies, depending on the family’s needs

  • In Hudson County, collaborates with Metropolitan Family Success Center, and the County Welfare Agency for referrals and submits a bi-weekly report of all welfare recipients and submits report to immediate supervisor by every Monday

  • In Bergen County, collaborates with Englewood or Holy Name Hospitals, and the County Welfare office

Education/Experience/Qualifications

  • Must have a minimum of a High School Diploma
