Key Responsibilities

Provides ongoing supportive services to consumers of the Transforming Lives program. Duties include but are not limited to assisting consumers with all aspects of the Social Security Entitlement Process [covering applications, reviews, appeals, etc.] The Benefits Counselor will provide personalized benefits planning, guidance on employment/earned income and time-limited case management to meet individual’s concrete and financial needs. Aids consumers with other benefit issues such as Medicaid/Medicare entitlements and enrollments/reviews, rental assistance, Welfare, Food Stamps, etc. Monitors consumer overall entitlement and eligibility. Serves as consumer advocate and liaison between outside agencies and community at large.

  • Maintains confidentiality in the daily operations of the department/Agency; immediately reports to management breaches of confidentiality.
  • Serves as resource to staff and consumers in regards to benefits and entitlements. Serves as liaison between various State and County agencies, ensuring consumers maintain eligibility, troubleshoot problems and advocates on their behalf. Transports and accompanies consumers to other agencies, when needed.
  • Monitors and reviews all new developments and regulations in regards to consumer rights, benefits and entitlements. Ensures consumers and staff are updated on relevant changes to regulations effecting entitlements.
  • Advocates and handles appeal for consumers in terms of benefits and other issues. Refers consumers for legal counseling when necessary.
  • Maintain files and databases as required.
  • Provides benefits/entitlement training to team and collaborative agencies/employers on a regular basis.

Qualifications/Experience/Education

  • Bachelor's Degree in Social Work or related field
  • Knowledge and experience working in the mental health field
  • Knowledge of State, County and Federal benefit and entitlement programs
  • Strong written and oral communication and listening skills
  • Computer, basic math and organizational skills
  • A valid driver’s license and must be insurable by agency’s automobile insurance

Starting at $40,000 annually
