We believe vaccination is the best protection for our patients, community and Visiting Nurse staff.   Location Lakeside   Category Home Health   Description The Registered Nurse- RN-Home Health Admissions (Sugarloaf) will demonstrate advanced clinical expertise and proficiency in OASIS completion and accuracy, development of patient specific care plans that target and promote patient independence in disease management and prevention of avoidable hospitalizations, and advanced proficiency in technical skills necessary for providing, and coordinating care to patients. Consults with and educates other care team members concerning applicable patient assessment and treatment protocols. Serves as a rehabilitation resource for the care team. Demonstrates advanced clinical expertise and proficiency in OASIS completion and accuracy. Must submit all required SOC documentation within 48 hours of the visit. Consistently meets or exceeds productivity goals. Participates in the orientation of new hires and participates in additional organizational activities such as QI, committee membership or serving as an acting assistant or Branch Manager.   Full-Time/Part-Time Full-Time   Position Requirements Education: Graduate of an accredited Diploma, Associate Degree or Baccalaureate School of Nursing. Preference is given to individuals with Baccalaureate Degree. Licensure/Certifications: Must have current licensure by the Georgia State Board of Nursing and a current valid Driver s license. Must have valid CPR certification. Certification as a COS-C preferred. Previous Experience: 2 years of home health required. Preference given to clinicians holding a national certification in a clinical field. See Tier requirements. Knowledge and Training: A professional knowledge of the theories, principles, practices and techniques inherent to the scope of practice. Communication skills to interact effectively with patient, family, care team members, referrals and community resources. Knowledge of accrediting bodies standards, Medicare, Medicaid and other funding source regulations as applied to home care services. Demonstrates competent techniques and practices of their profession.
