SUMMARY:

The Nurse HIV Clinical Care Coordinator is responsible for coordinating all aspects of care for HIV/AIDS patients. The Nurse HIV Coordinator assists the Ryan White HIV Program Director and Ryan White HIV Clinical Director in developing, implementing and maintaining programs and activities related to HIV primary care services.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Work closely with primary care providers and Ryan White HIV program staff to provide the following services to HIV/AIDS patients enrolled in ODCHC’s Ryan White HIV program at those locations housing ODCHC Ryan White Services Providers.

    • Develops teamwork and maintenance of good working relationships with co-workers.
    • Assesses physical condition of patient using observation, inspection, auscultation, palpation, and percussion.
    • Counsels client and family concerning health assessment and care.
    • Refers patient and families to other services or for follow-up treatment based on assessment of health care needs.
    • Develops a health care plan in cooperation with the patient/family, physician or other member of the team.
    • Conducts medication adherence program.
    • Screens for non-clinical service needs and make referrals to other agencies, as appropriate.
    • Provides screening and triage of patient phone calls and messages.
    • Maintains selected medical record flow sheets and other clinical data collection forms.
    • Coordinates and chairs interdisciplinary case conference meetings.
    • Ensures accurate collection and reporting of all information required to meet program and regulatory guidelines.
    • Performs HIV and AIDS reporting.
    • Provides screening and documentation of eligibility for special grants and programs.
    • Coordinates expenditures for selected special grants and programs.
    • Interacts with other HIV-AIDS agencies, and attends inter-agency team meetings.
    • Participates in evaluation and assessment of the Title III program, including quality assurance and patient satisfaction.
    • Participates in HIV-AIDS outreach and education activities.
    • Participates in Ryan White HIV Program needs assessment and strategic planning activities.
    • Performs related duties as assigned.
    • Adherence to the ODCHC attendance policy.
    • Other duties and responsibilities as designated by supervisor.

In addition, the Nurse HIV Clinical Care Coordinator may be expected to provide the above services, on a limited basis, for the Del Norte Community Health Center in Crescent City

 
Position Requirements

QUALIFICATIONS:

  • Ability to work as a member of a team in order to communicate and/or solicit pertinent information to and from other team members and support team decisions.
  • Ability to communicate effectively and exercise sound and responsible judgment.
  • Excellent interpersonal skills, written and verbal. Ability to establish constructive working relationships with all levels of management and employees in a staff of varied and diverse backgrounds.
  • Ability to effectively present information and respond to questions from groups of clinicians, support staff, clients, and the general public.
  • Ability to be persuasive and diplomatic in encouraging teamwork and cooperation in the development of ODCHC system-wide and multi-agency programs.
  • Ability to handle difficult or confrontational situations in a calm, consistent, and equitable manner.
  • Ability to read, analyze, and interpret business periodicals, professional journals, technical procedures, governmental regulations and epidemiological analyses.
  • Excellent organizational and time management skills.
  • Excellent computer skills, including ability to use word processing, data management and practice management programs.
  • Ability to work with minimal supervision as part of a team.
  • Ability to effectively represent ODCHC’s interests in the community.

EDUCATION and/or EXPERIENCE:

  • Degree in nursing leading to license and Registered Nurse State of California.
  • Preferred, but not required: Three years’ experience and strong clinical background in HIV/AIDS care, including knowledge of current HIV medical protocols.

CERTIFICATES, LICENSES, REGISTRATIONS:

  • Current RN license for State of California.
  • Current CPR certificate.

SUPERVISORY RESPONSIBILITIES:

NONE

PHYSICAL REQUIREMENTS:

  • Speaking and hearing sufficient to communicate effectively by phone or in person, at normal volumes.
  • Vision adequate to read correspondence, computer screen, forms, etc.
  • Good manual dexterity.
  • Ability to sit or stand for extended periods of time.
  • Ability to lift up to 40 pounds, to provide clients with supplies and assist those in wheelchair or limited mobility settings.
  • Must have valid California Driver’s license and a reliable form of transportation.
