The Family Resource Network is a comprehensive family-focused umbrella of organizations designed to meet the growing need for community-based programs and services to individuals and families with a variety of disabilities and chronic conditions through our three affiliate organizations. The Family Resource Network is a statewide organization that serves approximately 30,000 individuals annually. The Coordinator Assistant works in partnership with Support Coordinator to manage a caseload. Ensures qualified individuals with disabilities and their families identify and obtain the needed services and support to improve their quality of life within the home and in the community.

Coordinator Assistant Responsibilities:

  • Works closely supporting Support Coordinators.
  • Answers in-coming phone calls, when needed.
  • Provides the Support Coordinator with appropriate referrals for initial home visit based on the initial phone call information collection.
  • Reviews all documents, records notes and updates/uploads all records in required format (outlook, foothold, etc.) as needed.
  • Maintains continuing contact with individuals and families as individual plans of care are put in place.
  • Assists individuals and their families in coordinating, selecting and overseeing their own supports.
 
Position Requirements
  • Bachelor’s Degree
  • At least one year of experience working with individuals with intellectual or developmental disabilities.
  • Proficient in MS Office Outlook, Word, Excel, Power Point;
  • Able to solve problems independently, effectively and creatively.
  • Excellent Organization and Communication skills;
  • Excellent time management skills to be able to juggle multiple plans for multiple families simultaneously.
  • Must have a flexible schedule to meet the needs of our customers;
  • Bilingual strongly preferred.
