Case Manager - Justice Involved Services 


Key Responsibilities:

Provides direct case management services to an adult mental health population with legal issues in the community on a 24/7 hours basis. Spends 65% of time face-to-face with consumers, in their homes and the community. Assures departmental goals or objectives are accomplished within prescribed time frame by performing the job responsibilities personally.

  • Assesses/evaluates consumers and families at intake/admission, during ongoing treatment/service delivery and at discharge; collaborates with treatment providers to establish comprehensive assessments and service plans
  • Performs the following duties as a member of an interdisciplinary team (internal and external) - conducts outreaches, assesses consumers and articulates (verbally and in written documentation) the integration of consumer history and assessment information; conducts individual, group and family counseling; provides and or receives clinical feedback; monitors and maintains team caseload; mentors new staff
  • Formulates psychological service plans containing relevant goals, behaviorally measurable objectives (that serve as indicators for evaluating progress) and realistic time frames for achievement; performs quarterly reviews of progress and evaluates effectiveness of goals/objectives
  • Engages consumers in services and accesses/addresses their needs; assesses medication compliance; provides substance abuse services, family consultation, crisis intervention and vocational/educational/employment services; assists consumers in completing appropriate financial/insurance forms; includes client feedback in all interventions and utilizes a strengths-based perspective; provides comprehensive services based on Risk of Hospitalization assessment
  • Records the following information in the consumer's clinical record; assessments, service plans, progress notes (DAP format); linkages; statistics; and discharge summaries, with all entries authenticated and dated
  • Works with administrative staff to ensure that all services are documented based on billing and regulatory standards; collects information on consumers' insurance and reflects all demographic data
  • Monitors team functioning to ensure consumers receive required level of care; researches and coordinates resources for service provision and education/training; assists in the development and implementation of educational groups
