With the belief that all people have the capacity to grow and change, we provide our services to people of all ages, cultures, races, religious preferences, genders, and sexual orientations in order to enhance wellness and recovery, thereby improving families, workplaces, and communities.   Position RN - HIV Prevention   Description New opportunity for a full-time HIV Prevention and Sexual Health RN Care Coordinator to engage new and existing patients for HIV risk assessment, rapid HIV testing, HIV prevention, and HIV treatment services in our comprehensive HIV care continuum. The qualified applicant will also identify those at risk, increase HIV testing, expand access to HIV pre-exposure prophylaxis (PrEP) services, link HIV positive patients to treatment with our internal HIV treatment providers, and follow PrEP, HIV positive, Hepatitis C positive patients to ensure follow up and adherence to the treatment plan. Approximately 50% of the time for this position will be spent at community partner sites doing patient outreach, engagement, rapid HIV and STI testing, and PrEP follow up. The other 50% will be office-based engaged in care coordination activities with patients in person or over the phone to support PrEP, HIV positive, Hepatitis C positive, and STI positive patients.   Position Requirements AA/BA required; Experience working in a community-based setting preferred. Experience working with high-risk patient populations including those with several mental illness, substance use disorders, those experiencing homelessness, and engaging in sex work, preferred.     Location Southeast, Inc.
