Assure implementation and coordination of the plan. Oversee the development and implementation of the Individualized Family Service Plan per Act 212 And EI Regulations. Monitor client programs as per established early intervention requirements. Maintain records, correspondence, service plans and authorizations for each client. Make necessary referrals and coordinate services as outlined in the Service Coordination Support Plan (including other medical and community supports). Have weekly supervisor conferences on specific cases on caseload. Maintain established SC performance expectations. Prepare monthly case management statistics. Attend staff conferences of other agencies as they relate to early intervention supports and services. Assure close working relationship with other human services agencies as they relate to assigned caseload. Initiate and assist with transition to the Intermediate Unit. Close cases no longer necessitating early intervention services. Assist with covering cases when position vacancies exist in the unit. Other duties as assigned by Supervisor.   Position Requirements BSW which includes 12 college credits in early intervention, early childhood special education, early childhood education, child development, special education, family counseling, family studies, social welfare, psychology, or other comparable social sciences. 1 year of full-time or full-time equivalent experience working with or providing counseling to children, families or individuals with disabilities.
