Location: Home Base be in our Newark Center/Licking County, but also covering nearby counties: Perry, Hocking, Fairfield, and Morgan.

Schedule: Generally 8-4:30, however, time is according to the needs of the families served.

What You Would Be Doing
Primary job responsibilities involve service linkage and care coordination to assist consumers in addressing their behavioral health needs and tracking relevant health outcomes. Case Managers will provide community support, case management and service linkage both within Boundless and with outside providers that identify and address behavioral health needs, developmental issues and barriers to care. Care Coordinators will work with consumers, parents/guardians, Boundless staff and outside providers that support treatment and contribute meaningfully to whole health outcome of the individual served.

  • Provides case management support to consumers engaged in behavioral health services including community psychiatric support and therapeutic support to promote the coordination and implementation of treatment goals
  • Serve as a liaison between families and agency to positively promote collaborative processes in the treatment planning for clients by encouraging parent/guardian participation in treatment.
  • Identify and assess psychosocial and life skills needs through analysis of factors impinging on client success including factors in the home, school and community.
  • Provide parent/guardian educational workshops on identified issues related to child development, stress reduction, discipline and safety, and teacher/parent/client communication.
  • Keep caseload spreadsheet (information management) updated regularly.
  • Manage phone calls from families, guardians, agencies, etc. Set up meetings; obtain information, etc.
  • Provides case management (coordinate and link all services) for clients and families.
  • Develop programs to address parent/guardian participation in agency services and client engagement in the treatment process.
  • Maintain all required clinical records and submits appropriate documents for statistical reports with adherence to program standards.
  • Locate and mobilize community resources to support the treatment.
  • Attend meetings and professional development activities as required.
  • Provide 22 hours of billable service each week.
  • All other assigned duties.
 
Position Requirements

Education and Experience Requirements:

  • Bachelor’s degree in social work, education, psychology, or another related field is preferred
  • High School and GED required
  • Two plus years working in Human Services required
  • Two plus years of experience providing case management services preferred
  • Experience working in the behavioral health field preferred
