SAFY of Findlay, Ohio is seeking a full time Intervention Specialist, responsible for comprehensive clinical service provisions for clients, as well as promoting family stability within a framework of strength based and culturally sensitive services.

As the Intervention Specialist you would:

  • Provide a range of moderately complex clinical services with children and adolescents. This will include diagnostic assessment, treatment planning, and individual, family and group therapy according to program specifics.
  • Participate in the development of a Program’s documented treatment/service plans and provide identified behavioral services to meet the need of the clientele and/or program specifics.
  • Provide short-term crisis intervention as needed/identified. Will follow intensive, evidence-based service models based on Trauma Informed approaches. Services are provided at the level of frequency, intensity and duration appropriate to program/ service design with a focus toward preventing out of home placement, achieve reunification, or meet the treatment goals as defined.
  • Develop/Maintain consultative and liaison relationships with catchments area caregivers (e.g., schools, human services agencies, etc.) in order to cultivate work relations, provide consultative services and/or promote the coordination and development of mental health services. Establish mutually respectful rapport and implement interventions necessary to meet needs of client/collaterals; appropriately set limits and maintain helping role as a Mental Health practitioner.
  • Prepare all required clinical documentation per established time frames involving all appropriate parties as required by policy and regulations of the state/ program/ contract/ SAFY policy and COA standards.
  • Address mental health concerns through education groups, individual and classroom consultations and interventions, team meetings, brief crisis management, family therapy.
  • Develop / assist in development of crisis, safety and behavioral management plans.
  • Provide instruction, intervention and assistance as deemed necessary through the exercise of independent judgment and knowledge or as part of the treatment team
  • Collaborate service provisions with school officials, courts and other appropriate parties involved with the client according to policy, regulations and best practice standards.
  • Provide services to the youth with mental health diagnosis; promote daily living skills that impact delivery of mental health services which focus on self-sufficiency through specific outcomes, such as obtaining housing, job education or vocational training, counseling and community networking; providing outpatient mental health case management services.
  • Complete assessments as required by state rules, agency policies, and/or contract requirements, which may include psychosocial assessments, Diagnostic Assessment Forms, and Independent Living Assessments
  • Prepare Individualized Treatment Plans according to established time frames involving all appropriate parties as required by policy and regulations of the state.
  • Establish mutually respectful rapport and implements interventions necessary to meet needs of client/collaterals; appropriately sets limits and maintains helping role as a Mental Health practitioner.
  • Coordinate home visits for the youth with all appropriate parties. Discuss all relevant issues pertaining to the youth in care with foster parent/s, youth, school officials, natural parents, placing agency and other appropriate parties involved with the youth according to policy, regulations and best practice standards. Complete inventories of property for each youth and ensure foster parents are updating inventories.
  • Attend hearings and semi-annual review for all youth assigned to caseload. Assist with enrolling youth in school and attends Individualized Education Plan (IEP) meetings. Assure that health care needs of youth are being realized including medical, dental, vision and hearing as well as special needs. Provide after care for 90 days or as required by contract.
  • Staff will participate on an On-Call rotation system as designed by the division or state.
 
Position Requirements

What you bring to this role:

  • Master’s degree in the field of social work, counseling, psychology, or other license-eligible related
    field of human services.
  • Licensure: OH – LSW or LPC
