Share Email Opening Apply Now Title Social Work Supervisor- Bluestone  Description Job Summary: The Social Work Supervisor provides psychosocial assessment, counseling, crisis intervention, and community resource assistance on a consultation basis to patients and families utilizing a variety of treatment modalities, including individual, family, group and specialized counseling. Essential Duties and Responsibilities: Conducts psychosocial assessment interviews with patients to identify problems requiring social work interventions and treatment plans. Provides counseling to impact problems related to environmental needs, family or interpersonal conflict, grief issues, and coping with physical and/or mental illness using a variety of treatment modalities. Intervenes in crisis situations. Assists patients in adjusting to episodes of acute and chronic illness. Provides consultation to other healthcare professionals when coordination with significant or intensive community resources is necessary to achieve desired treatment outcomes. Participates in treatment planning for each patient, works cooperatively as a member of the interdisciplinary treatment team; completes progress notes and discharge plans in a timely fashion. Collaborates with Utilization Review Specialist and other members of the healthcare team to provide for psychosocial assistance. Treats patients and families with respect and dignity. Identifies and addresses psychosocial needs of patient and family. Acts on performance improvement issues identified during CQI. Maintains line of communication between social workers, therapists, nursing staff, physicians and patients/families to ensure coordination of treatments for optimal patient care. Accurately determines type of assistance/setting/resources necessary for the patient/family and provides appropriate resources/assistance/list of facilities. Provide discharge planning, arrange for follow-up care and information exchange with sources outside of hospital. Monitors and evaluates the social work services furnished. Available days, evenings, weekends, and holidays a needed, Utilize verbal and physical de-escalation as warranted in order to keep patients safe and remain current in warranted certification and training. Other Duties and Responsibilities: Adheres to all Hospital policies Communicates effectively, both verbally and in writing Maintains in using basic computer programs, including the Hospital's electronic health record and other programs needed to complete the role. Completes annual education requirements. Attend scheduled staff meetings, supervision, and on-going training. All required trainings, certifications and licensure must be kept current in accordance with applicable licensing and accreditation regulations and standards. Maintains regulatory requirements, including all federal, state, local regulations and accrediting organization standards. Maintain high standards of ethical and professional conduct and adhere to Agency policies and procedures. Maintains and ensures patient confidentiality at all times. Attends annual review and performs departmental in-services. Works at maintaining a good rapport and a cooperative working relationship with physicians, departments and staff. Represents the organization in a positive and professional manner. Attends and participates in committee, CQI, Utilization Review and varying team meetings and other meetings as appropriate. Resolves staff concerns at the department level, utilizing the grievance process as required. Ensures compliance with policies and procedures regarding department operations, fire safety, emergency management and infection prevention and control. Effectively and consistently communicates administrative directive to staff and encourages interactive departmental meetings and discussions. Complies with all organizational policies regarding ethical business practices. Communicates the mission, ethics and goals of the facility, as well as the focus statement of the department. Other duties as assigned. Education/Licensure Requirements: Master’s Degree in Social Work (MSW), LCSW (Licensed Clinical Social Worker) and Licensed Independent Chemical Dependency Counselor (LICDC) is required. Facilitate Quality Assurance/Quality Improvement (QA/QI) functions to ensure investigator and institutional compliance with federal and state regulations. Accomplishing this by providing ongoing support, education and study management tools, and best practice recommendations that go above and beyond mere compliance, with an emphasis on improving efficiency. Five (5) or more years of previous experience in all aspects of social service with an emphasis in the behavioral health setting.
