Community First Health Centers is currently looking for a Behavioral Health Clinician; LMSW to join our team!! The ideal candidate for this role will be dedicated, detailed and a team player.

Under the supervision of the Behavioral Health Program Manager, you'll provide behavioral health consultations in the primary care setting to children, adolescents, adults and families.

This position will be housed out of our Algonac, MI medical facility located at 555 St. Clair River Drive. Flexibility will be needed to travel to our other locations (New Haven or Port Huron) and in the field working with community partners and building our patient base as directed.

Daily responsibilities of our Behavioral Health Clinician; LMSW may include:

• Assessing patient social, emotional, environmental and support needs
• Work with the patients family and other service providers agencies to develop a plan of care
• Complete and maintain all required data/documentation of services to clients for both case and Agency records including referrals, intake, treatment plan, progress notes, status reports, case management as well as closing and follow up services
• Submit timely/accurate client daily activity logs/encounter forms for billing/statistical purposes
• Provide individual and/or group therapy and/or crisis intervention when needed
• Provide education on issues of mental health to service recipients and families
• Assist in the development and integrating of mid-level services into the clinical delivery model of the Agency, communities and partnering hospitals
• Participates in quality improvement activities
• Other duties as assigned


This is a full time; 40 hours per week opportunity.

Hours will be gained predominately 830am-5pm, Monday thru Friday with some flexibility based on business demands.

Position Requirements

• Bachelors and/or Master Degree in Social Work or Counseling
• Master of Social Work (MSW) or obtainable within six (6) months of hire
• Licensed Professional Counselor (LPC)
• State of Michigan Licensure as a MSW, LMSW or Licensed Professional Counselor – Limited license acceptable if supervision is available
• Experience working with behavioral health recipients/issues
• Valid State of Michigan Driver’s License in good standing
• Reliable transportation with current auto insurance
• Experience with an EMR system with above average computer proficiency
• Experience working with a diverse population; specifically those in need/with limited resources
• Ability to work independently and with minimal guidance
• Excellent interpersonal and customer service skills
• Must maintain Basic Life Support certification
