Description

JOB SUMMARY: Provides an array of mental health support services to the PACT consumer. Participates in the planning and provision of interdisciplinary training and development for all team members. On-call availability and response is required.

ESSENTIAL FUNCTIONS:

  1. Provides support services which include but are not limited to personal care assistance (hygiene, grooming), life skill development (nutrition, money management, medication monitoring), entitlement management and advocacy, and supportive counseling (emotional support, job coach/vocational support, substance abuse support). Coordinates services with other agencies.
  2. Participates in treatment planning, ongoing assessment and discharge review process.
  3. Responsible for maintaining accurate and current consumer records.
  4. Advocates with community service providers for on-going, emergency and other forms of health care for consumers.
  5. Represents PACT, the Agency and/or consumer at various community based medical, treatment planning and service coordination meetings.
  6. Contributes to and participates in the delivery of interdisciplinary in-service training and case conferences.
  7. Provides on-call and emergency response services
  8. Provides psychiatric/medical case management and/or specialized services to consumers as warranted and/or requested.
  9. Participates in on-going supervision and/or program development with the Program Coordinator, which may consist of field visits to consumer residences, work sites, families, etc.
 
Position Requirements

- Individuals recovering from a mental illness who have been psychiatrically stable as verified by their treating physician/program and who have received psychiatric/medical clearance from their treating physician/clinician to return to work on a limited to full time basis will be considered.

- A High School Diploma + 2 years experience (work or life) in the mental health field is required. (An Associates Degree in Psychiatric Rehabilitation; or a Bachelors Degree in Behavioral Science is preferred).

- Valid Drivers License.

- The ability to be on-call and provide emergency response required. - Bi-Lingual capacity (Spanish/English) desirable.

Weekly Work Hours 40 hours per week  
Proposed Salary $35,000/ $25 on call rate  
Schedule Mon-Fri 8am-4pm. Occasional weekend hours.
