Job Summary: The CPSW has a thorough knowledge of basic Medical and Perinatal terminology, symptoms and procedures. He/she will have experience working with a multi-disciplinary team, good communications and interviewing skills. The CPSW will also be able to accept and work with people who have a diverse range of literacy and verbal skills. He/she will be sensitive to various family compositions and possess the ability to work under stress (i.e. with a high case load (14+ patients per day) and with individuals in crisis).

Essential Functions and Responsibilities

  • Conducts health education and case management for CSVS obstetric patients per policy guidelines (Calif State CPSP Program and CSVS Protocols)
  • Conducts CPSP orientation, initial assessments (nutrition, health education and psycho-social), trimester reassessments and postpartum assessments.
  • Provides patient education on prenatal care, baby care and safety, family planning and labor and delivery.
  • Completes all referrals and forms as required (WIC, Mental Health, Dental etc.).
  • Develops an individual care plan for each patient.
  • Maintains appropriate documentation of each patient visit in the electronic medical record including the time spent, educational materials, patient handouts and audio-visual materials viewed.
  • Schedules patient appointments. A minimum of 14 patient visits per day.
  • Measures height, weight, and blood pressure at each visit documenting on the correct grid for each patient. He/she will report any abnormal values immediately to the provider of record.
  • Orders correct laboratory tests and studies in a timely manner per CSVS Protocols. Abnormal results are reported to the provider the day of receipt.
  • May draw blood for special analyses when indicated.
  • Orders special test during as required.
  • Revises the patients care plan at least quarterly. The plan will include patient concern/request and a CPSW/provider identified concern.
  • Request additional Medical visits for High Risk visits.
  • Alerts the provider when the allowable visits (10) are being used rapidly. Offers CPSP hours with education (8min min.) as an alternative.
  • Answers patient’s questions and concerns within scope of practice.
  • Files lab and ultrasound reports as needed. Checks the Roseta Tank for missing reports or information.
  • Orders health education materials from approved resources when needed.
  • Provides feeding information on risk/benefits for breastfeeding, community resources for support and referrals at any time throughout the pregnancy/postpartum period.
  • Organizes and maintains the CPSP office. Only approved materials may be posted on the walls.
  • Discourages the purchase of breast feeding accessories (inc. pumps) unless approved by the provider or Lactation Consultant. Will refer vendors to the PSM/Administration office.
  • Does not endorse any specific product for purchase unless approved by CSVS.
  • All patients charting is completed daily.
  • Maintains UDS documentation daily and infant tracking system.
  • Makes outside referrals as necessary.
  • Maintains confidentiality of patient information.
  • Review all patient charts with the primary provider at a minimum of once a quarter for routine patients. High risk patients monthly or on a as needed basis.

  • Maintains vaccinations for MMR, Tdap, Flu and annual TB testing.
  • Assists patients without prejudice or bias.
  • Complies with all policy and procedures as outlines in the Calif St CPSP program and CSVS manuals.
  • Complies with all laws and regulations in the delivery of healthcare and interactions and the administration.

  • Renew CPSP training every 3 to 5 years. Complete any program updates as needed.

     
    Position Requirements
    • EDUCATION
    • High School Diploma or equivalent
    • CPR/BLS certification
    • Medical Assistant Certificate and/or National/State Medical Assistant certification
    • Language Required: Bilingual Spanish and English
    • Work Experience: One year medical office experience in Obstetrics

    One year in Birth Control a plus
