SAFY of Montgomery, Alabama is seeking a mission driven professional to serve as a full time Independent Living Case Coordinator responsible for the case management functions of families and youth in our division.

As an Independent Living Case Coordinator, you would:

  • Provide In-Home, case management, and rehabilitative services to older youth in transitions to adulthood. The typical caseload is 8 youth.
  • Provide instruction, intervention and assistance as deemed necessary through the exercise of independent living skill, copying skill, health managing, job skills, social skill, crisis management, and behavioral management.
  • Prepare Individualized Treatment Plans, PSD's, and Mental Health Services Plans according to established time frames involving all appropriate parties as required by policy and regulations of the state.
  • Create documentation for all services you provide in our Electronic Health Record System. Timely document (collaborative or within 24 hours) outcome of home visits, support sessions, interaction with parties involved in the youth's care and incidents as required by policy, regulations and contract requirements.
  • Complete referral reviews and intake assignments. Communicate service availability to all assigned clients promptly.
  • Participate in rotating on-call schedule with the other team members. Generally, we're on call once every other month.
  • Develop and maintain consultative and liaison relationships with DHR and other members of the youth’s treatment team (biological parents, school, etc.)
  • Travel throughout Montgomery and surrounding counties. Overall hours being worked outside of the office is estimated to be around 60%, which includes traveling to and from visits and time spend at the home conducting a home visit.
  • Set your schedule in collaboration with the team. Generally, we work 40+ hours a week based on the availability of the families we serve. There may be times when you may need to work until 8pm or late. Your supervisor will work with you to flex time as needed
Position Requirements

What you bring to this role:

  • Bachelor’s degree in the field of social work, counseling, psychology, sociology, child & family studies, or other approved related field of human services
  • Two years of relevant experience with above listed degrees, experience in child welfare preferred, OR relevant experience in working with older youth services in general.
  • Three to five years of relevant experience required with related field of human service degrees
