Concern is seeking a certified Peer Specialist to provide Home and Community Based Services (HCBS) on a per diem basis to HARP enrolled eligible tenants/ residents in our supportive housing programs across Brooklyn and the Bronx. Applicants should be individuals with lived experience as a consumer of mental health services who are willing to assist others in their recovery and rehabilitation in the community. Services will be delivered within the home or at convenient locations in the community. Duties and Responsibilities: Provide strength-based rehabilitative and recovery oriented services as part of peer-to-peer evidence-based practices under HCBS. Be willing to share personal experiences to foster sense of hope and recovery. Provide face-to-face individual services in a client's home or community following the HCBS protocols, documenting all services in the Electronic Health Record (EHR) using collaborative documentation. Carry a caseload of four to eight clients. Ensure coordination of services through collaboration and consultation with other staff, providers and identified family and/or friends of the tenant/ resident. Assist tenants/ residents with entitlements and concrete services; working together and advocating on their behalf via telephone and written correspondence. Attend and participate in weekly supervision, staff meetings and periodic in-service trainings, including developing competencies in evidence-based practices. Document time and effort and tasks during working hours as required for billing. Remain current with all Agency required compliance documents and trainings. Other duties as assigned.   Position Requirements High School Diploma or GED and OMH or OASAS approved peer certification along with two years life experience with behavioral health issues. Experience with advocacy, outreach and engagement, self-help, recovery and other local support systems. Knowledge of Evidenced Based Practices (Motivational Interviewing, Harm Reduction). Bi-lingual Spanish/ English preferred. Experience with Electronic Health Record (EHR ) a plus.   EOE Statement We are an equal employment opportunity employer.
