All qualified applicants will receive consideration for employment without regard to race, color, religion, sex, national origin, disability status, protected veteran status or any other characteristic protected by law.   Description The responsibilities of the Care Manager will include ongoing contact with assigned consumers, in-home reassessment of consumer needs on a regular basis, completion of the consumer care plan, revision of consumer care plan as needed, monitoring of service delivery, encouragement of family involvement, and advocacy on consumers’ behalf. Additionally, clerical and appropriate documentation of aforementioned activities in an electronic consumer records database are two daily responsibilities.   Position Requirements BSW with experience coordinating assigned services as part of the client’s care or treatment plan, teaching client living skills, aiding in therapeutic activities and providing socialization opportunities for clients.
