This position will make referrals and coordinate the needs of patients to assure high quality care. Schedule: Monday through Friday, ~4-8pm.     Position Requirements Must have an AA/BA or Community Health Worker certification. Prior healthcare experience preferred. We are a healthcare facility and as such we require that our new employees are fully vaccinated (both COVID and FLU). Additionally, to promote a healthier workforce, we only hire non-smokers.
