Case Manager - Intensive 

Description

The Intensive Case Manager provides intensive community-based mental health services to adults who require multi-systemic coordination of services to successfully remain out of the hospital and jail, and in the community.

Essential Job Duties:

  • Provide community based intensive case management services to adult consumers with a mental illness and/or co-occurring substance use disorder who have frequent contact with criminal justice entities.  These individuals are identified primarily through law enforcement (CIT), jail diversion coordinators, and municipal and district court partners.
  • Works with team members, community partners, and service organizations to provide direct-service to at-risk adults who have involvement with multiple community organizations and municipal agencies. Direct services may include, but are not limited to: crisis intervention, assessment, mobile outreach, travel with clients to appointments and court-hearings, assist with applications for benefits like Medicaid and disability, housing, employment, job development skills and system navigation skills.
  • Assists with the development of diversion plans to help reduce/prevent hospitalizations and incarcerations; actively participate in team planning for implementation of diversion plans.
  • Acts as a boundary spanner for criminal justice systems, mental health systems, and additional community based service organizations. 
  • Monitors program data and develops monthly reports.
  • Completes timely and accurate documentation of services in electronic medical record.
  • Actively participates in the research, development and implementation of emerging multi-system models and practices.
  • Coordinates community / mobile emergency responses in conjunction with the Emergency & Stabilization Services team as needed.
  • Coordinates with 24/7 crisis line contractor as needed.
  • Confers with various Center staff to ensure optimum consumer care.
  • Models and inspires flexibility and creativity in the delivery of clinical services.
  • Outreach individuals who are in need of mental health services and have had frequent law enforcement contact.
  • Prepare court letters for individuals on Out Patient Treatment Orders through wellness court.
  • Prepare revocation documentations for individuals who are non-compliant with Out Patient Treatment Orders through wellness court.
  • Assures staff and program compliance with Medicaid, State licensing regulations, and other contractual standards.
 
Position Requirements

Job Requirements:

  • Bachelor degree from an accredited University, with preferred experience in the human service and/or criminal justice fields.
  • Excellent interpersonal and communication skills, including both oral and written.
  • Ability to establish effective working relationships with both Center staff and various community agencies.
  • Ability to facilitate planning meetings with individuals from multiple systems.
  • Proficient in use of technology, including Microsoft Office.
  • Provide and maintain dependable transportation and auto insurance.
  • Ability to build and maintain close working relationships with clients and community partners.
  • Relevant experience in mental health emergency services.
  • Emergency Stabilization Services requires strong team skills with a positive spirit, high energy and a “can-do” personality.

Hours Monday - Friday, 8:00 am - 5:00 pm  
Pay Range $33 -$35k
