Social workers will be responsible for: Delivering social services to residents and their families Preparing social histories and participating in resident care conferences Completing care plans and MDS documentation Assisting with out of facility care including transportation Coordinating discharge plans and out of facility care if needed including transportation   Position Requirements Candidates must have a bachelors and/or masters degree in social work from an accredited school of social work and be licensed in Missouri (preferable) and/or Kansas.   This position is currently not accepting applications.
