Share Email Opening Title Case Manager - Wrap Around Facilitator - Youth  Description Wyandot BHN is seeking a dedicated Wrap Around Facilitator to join our youth case management team! The Wrap Around Facilitator is responsible for the provision of SED Medicaid Waiver services to children/adolescents and their families. Essential Job Duties: Facilitation of Wrap Around planning for Wyandotte County SED children/adolescents in averting hospitalization, or following discharge from the hospital and development of Waiver Plans of Care. Maintain case load of all SED Waiver clients. Responsible for oversight of the SED Waiver Plans of Cares for initial, 90 day reviews, and annual reviews. Development of individual and family strengths assessment and goal plan. Development of crisis plans Resource location/acquisition; and resource coordination with area agencies. Direct involvement in planning and advocacy in areas of: Education/Vocation, Residential, Financial, Health, Leisure, and Social Supports. Provision of supportive counseling to SED children/families in development of strengths/supports, skills development, decision making, and problem solving. Education to family in symptomatology, awareness of relapse indicators and handling of crisis. Assists in collection of program data and provides reports as necessary to immediate supervisor.   Position Requirements Job Requirements: Will possess a bachelor degree in a social services or other related field. Will possess the minimum qualifications as documented in the Kansas Case Management Program Standards for Children and Adolescents (on file). Ability to communicate effectively, orally and in writing. Ability to establish effective working relationships with staff, community agencies and clients. Ability to recognize and be sensitive to cultural and ethnic differences.
