Position Transition Youth Specialist  
Description

Our Transition Youth and Young Adult Services team at Southeast Healthcare Services is seeking a dedicated and qualified professional to provide community-based services to youth ages 14–18. This role is focused on supporting participants as they develop and strengthen independent living skills and employment readiness.

Key Responsibilities:

  • Conduct initial and ongoing assessments of participants’ independent living skills, vocational strengths, and interests
  • Develop individualized service plans aimed at achieving participants' goals in areas such as education, employment, and daily living
  • Provide hands-on support and coaching to assist youth in obtaining and maintaining employment
  • Facilitate access to resources and create learning opportunities that promote self-sufficiency
  • Collaborate with service providers, families, and systems of care to ensure coordinated support

Salary range based on education and related work experience = $35,000 - $42,660.

 
Position Requirements
  • BA/AA degree required
  • Experience working with transition-age youth
  • Strong understanding of independent living and vocational support services
  • Ability to engage youth in a strengths-based and goal-oriented manner
  • Excellent organizational and communication skills
  • A commitment to empowering young people toward greater independence
  • Willing to transport youth in personal vehicle as needed
