The CCM shall be responsible to support adults with disabilities of all ages and others who need assistance to lead self-directed lives and contribute to their communities; and supports behaviors that enhance inclusion in their communities.

The CCM is responsible to assure that quality supports and services are provided to people and that the philosophy of OHI is adhered to at all times. The CCM is responsible to assure that there is consistency in the supports provided to each person with disabilities; that the communication among team members is effective; that duplication of efforts is eliminated; bureaucracy is reduced; that the small agency feeling is maintained; that a holistic approach is provided to people supported; and best practices are used.

The CCM is responsible for teaching daily living, communication and work skills, providing safe transportation, assisting with or coordinating medical and other support service appointments, maintaining accurate and detailed documentation and records, and implementing the philosophy, policies and procedures of OHI.

 
Position Requirements

The Community Case Manager (CCM) shall have a minimum of a Bachelor’s Degree from an accredited four-year institution specializing in psychology, mental health and human services, behavioral sciences, social work, behavioral health, special education, counseling, rehabilitation, nursing, or a closely related field, and one year experience supporting people with intellectual disabilities.

The CCM shall have demonstrated ability to support people with disabilities; ability to work a flexible schedule; crisis prevention, intervention and teaching skills; and a valid Maine driver’s license and own vehicle.

About the Organization OHI is a Maine-based, public nonprofit organization providing supports and services to people with intellectual disabilities, autism, and mental illness for over 43 years. With offices in the greater Bangor area, specifically in the towns of Bangor and Brewer, OHI currently supports about 400 people in six Maine counties. OHI provides 24-hour residential services and various community services for people to live productively in their communities.

OHI also provides permanent housing in Bangor and Brewer for adults with disabilities, and professional development training for people, including family members, caregivers and support workers. In November 2013, OHI opened the Brewer Area Food Pantry as both a means to meet a growing need and to give back to a community which the people it supports rely upon.
OHI strongly believes in providing person-specific intellectual disability and mental health services tailored to the specific needs and desires of the person supported.
