We currently have an opening for a Full-Time experienced MDS Coordinator (RN). The role of MDS Coordinator is unique and requires someone with a minimum of one year of experience in care planning, assessment and the entire MDS/RAI process. Critical thinking, leadership skills, flexibility, excellent communication skills, and a focus on team building is a must.

This position is typically Monday through Friday during day shift hours, with some weekends and holidays.

Position Requirements

Qualifications and Experience:

  • Graduate of accredited school of nursing
  • Current state licensure as a Registered Nurse in good standing
  • Minimum One year nursing experience providing direct care in long-term care setting
  • One year prior charge nurse or similar supervisory experience; competent in leadership and communication skills
  • Minimum one year of experience with MDS/RAI
  • Ability to plan, develop and implement nursing policies and procedures
  • Demonstrated ability to interact effectvely, professionally and positively with all residents, staff and other healthcare providers, families and the public
  • CPR certified
  • Must be English literate and possess excellent written and verbal communication skills

Essential Functions of the Job:

  1. Working knowledge of Medicare rules and procedures, with expertise in Medicare documentation.
  2. Proficiency in computer data entry and retrieval.
  3. Knowledge of nursing principles (geriatrics, long-term car, rehabilitation, and acute medical care) and professional standards of nursing practice, and ability to apply to residents' specific circumstances. Able to identify, implement, and evaluate appropriate objectives and interventions for residents.
  4. Demonstrated competency in clinical skills needed for facility resident population.
  5. Demonstrated competency in physical, behavioral and functional capacity assessment of long-term care residents.
  6. Working knowledge of laws and regulations that influence provision of care and services in nursing facilities.
  7. Working knowledge and ability to interpret and demonstrate facility policies, procedures to direct care staff.
  8. Ability to teach small groups of nursing staff in classroom and on unit.
  9. Sufficient organizational and interpersonal skills to assign, motivate staff, elicit work output, improve quality and interact effectively with residents, families, interdisciplinary team members, administration, government officials, consultants and others in addition to staff.
  10. Carries out all duties in accordance with the facility mission and philosophy.
  11. Demonstrates knowledge of medications and their correct administration based on age of the patient/resident and their clinical condition. Evaluates patient/resident responses to medications.
  12. Observant for significant changes in resident's condition and communicates this to the physician and IDT for the purpose of the MDS.
  13. Demonstrates and is knowledgeable of the importance of maintaining confidentiality of resident and facility information.
  14. Plans for and provides patient/resident, family and caregiver education, identifies learning needs, clearly communicates information and documents patient/resident response to education.
  15. Able to understand and follow written and verbal directions. Able to express self adequately and accurately in written and/or oral communication (including documentation in clinical records), with direct care staff, interdisciplinary team members, administration, and government officials.
  16. Must have organizational skills, ability to assess, plan and implement and evaluate patient/resident care. Must be able to demonstrate ability to handle multiple priorities and the ability to direct and evaluate the activities of others.
  17. Sufficient mobility and strength to move freely through the building to assure resident safety at all times and to assist, transfer, or otherwise move residents of facility out of danger in case of emergency.
