Under the direction of the Social Services Director, you will assist in planning, organizing, implementing, evaluating, and directing the overall admissions operation for the facility. As a CNA/LPN or Licensed Social worker, this position will be a member of the Social Services Department and work in accordance with current federal, state, and local standards, guidelines and regulations, and our established policies and procedures to assure that the medically related emotional and social needs of the resident are met and maintained on an individual basis.


  • Complete admissions process and paperwork with Residents or Resident Representatives.
  • Provides explanation of any changes in rates and charges to those requesting a change of accommodation; assist with arranging for the change as appropriate.
  • Assist in providing solutions for social and practical environmental problems, including seeking financial assistance, such as the completion of a Medical Assistance application or Family Care enrollment.
  • Assist with facility private pay funds collections efforts, as directed.
  • Gather Social History and Discharge Observations to assist Interdisciplinary team with ongoing resident needs assessments.
  • Assist interdisciplinary team with the processing of all admissions and discharge care coordination efforts as well as conduct other interdisciplinary care planning observations, as directed.
  • Assist in the completion of insurance authorizations.
  • May assist Administrator in contacting families after admission and discharge to personally determine satisfaction.
  • May assist with problem solving as well as timely follow-up on family or guarantor complaints
  • Conduct facility tours and return phone calls to potential customers interested in rehab or long-term care placement, informing Marketing Director of any interest that may arise from this work.
  • Develop relationships with key referral sources within the community.
  • Participate in facility surveys and inspections made by authorized government agencies as directed.
  • Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the facility.
  • Attend and participates in scheduled in-service training, educational classes and meetings
  • Performs miscellaneous tasks as assigned.
  • If certified as CNA/LPN, assist as a CNA/LPN as appropriate

  • Minimum one-year experience in processing admissions and/or insurance authorizations to coordinate resident placements
  • Ability to make independent decisions
  • Demonstrates exemplary interpersonal relationship and communication skills, providing outstanding customer service.
  • Must be knowledgeable of administrative practices, procedures and guidelines
  • Excels at managing multiple priorities
  • Must possess the understanding and ability to comply with Federal and State rules and regulations as it applies to federal and state nursing home regulations (42 CFR Part 483, DHS 132, and DHS Chapter 50) Nurse Practice Act; and maintains CPR certification if licensed nurse.
