What are we looking for in a Case Manager Assistant?

Case Manager Assistants in our PACT (Program for Assertive Community Treatment) program provide support and assistance to the treatment team to ensure efficient, effective services are available at all times to our PACT clients. PACT is a service delivery model with the primary goal of recovery through community treatment. Its target population is those with challenging and persistent mental health issues. The PACT model is characterized by a team approach, shared caseload, time-unlimited services, flexible service delivery, and 24/7 crisis availability. The Case Manager Assistant provides stabilization and intensive support to our clients by offering backup to the primary Case Managers. Excellent communication and relationship-building skills, knowledge of community resources, and a willingness to travel locally throughout the workday are necessary in this role. The schedule for this position is Monday - Thursday 7am - 6pm, plus participation in the on-call rotation (approximately one week every eight weeks).

 

Case Manager Assistant duties may include:

  • monitoring clients who are awaiting services and providing updates to team
  • providing outreach and transportation as requested
  • participating in team staffings and assisting therapists & case managers with treatment planning
  • monitoring clients’ progress, symptoms, and medication response
  • assessing clients for safety risks and taking appropriate crisis intervention steps as necessary
  • assisting all team members as requested, which may include completing case-related paperwork, or making phone calls
  • completing progress notes and all required documentation
 
Position Requirements

Education & Experience: Associate degree in social services or related field and six months of relevant experience or equivalent combination of education and experience

Professional Credentials: Eligible for Agency Affiliated Counselor registration

Other Requirements: Driver's license & vehicle liability insurance
