Director of Care Management

The Director of Care Management functions primarily as the senior manager of the new Care Management service line. This is a leadership position responsible initially for starting the new service line by working with the executive sponsor and a cross-functional team to design, implement and manage the new service line. At least five years of experience in a care management leadership position is required.

The ideal candidate will have a mix of entrepreneurial, clinical and business experience and have a passion for searching for new ways to drive better health outcomes in a cost-effective manner using information technology, analytics and care innovation. Knowledge of how to select and maximize the value of care management software platforms will be essential. He/she should have deep experience in either the physical medicine care management and/or behavioral health care management and the ability to integrate the physical and behavioral care of beneficiaries with significant disabilities or mental health challenges. Preference will be given to candidates with constructive energy, strong leadership, planning, communication and collaboration skills, high integrity and good management capabilities.

The Director will be expected to help establish and then managed a successful service. The management approach should not be static but one that continuously innovates and learns from other industry leaders and innovators to adopt new program ideas, technologies, tools and management practices to achieve better data proven outcomes and cost of care efficiencies.

Essential Job Functions:

  • Co-lead CMA application process with the executive supervisor to obtain CMA certification in 2020, with support from a cross-functional team
  • Collaborate with manager, internal partners, third party entities, regulatory officials, consultants, vendors, etc. to design, establish, build a highly competitive CMA to start operations by July of 2021.
  • Recruit and hire care management supervisors, care managers and other service line employees
  • Develop policies and standard operating procedures for the care management service line
  • Develop and execute appropriate management processes and tempo to achieve quarterly and annual goals
  • Continuously seek to improve outcomes and cost-effectiveness of care for beneficiaries under management, targeting a best-in-class performance and continuous improvement
  • Oversee scheduling of care management team and affiliated support teams so as to provide, at minimum, contractually mandated service levels
  • Collect, review and publish quality and performance metrics and report status on a monthly basis
  • Intrastate travel to visit care management offices and observe and coach care management staff
  • Attend appropriate professional conferences to build your professional network and maintain expertise
  • Design and maintain a robust education and training program for care managers and affiliated staff
  • Ensure all legal and regulatory requirements are met and achieve required certifications and licensures
  • Ensure compliance to the North Carolina Provider Manual for Tailored Care Management and other state requirements
  • Champion and promote the care management service line internally and externally, working with marketing colleagues to market and grow the care management customer base
  • Work with the executive supervisor to manage the service line P&L to annual financial targets
  • Follow all ESUCP procedures as appropriate to position

Supervisory Responsibility:

The Director of Care Management will directly manage multiple care management supervisors in the daily operations of the care management team. He/she will directly and/or indirectly supervise other support personnel in the care management service. He/she will have the ultimate responsibility to ensure high-quality care management operations.

Position Requirements

Minimum Qualifications:

  • Master’s-level fully Licensed Clinical Social Worker (LCSW), fully Licensed Professional Counselor (LPC), fully Licensed Psychological Associate (LPA), or a registered nurse with a Bachelor of Science in Nursing (BSN); and a Bachelor’s degree in a human services field (including nursing) and five years of supervisory experience working directly with complex individuals with I/DD or TBI; or a Master’s degree in a human services field (including nursing), with three years of supervisory experience working directly with complex individuals with I/DD or TBI
  • Five years of supervisory experience working directly with the population being served.
