Bilingual Spanish a Plus

The Support Coordinator helps individuals with disabilities and their families to identify and link with needed services and supports to improve their quality of life within the home and the community. Responsibilities includes but are not limited to:

  • Develops individual plans of care, with input from the individual and family, which is based on the personal preferences and choices of the individuals being served.
  • Maintains continuing contact with individuals and families as individual plans of care are put in place.
  • Assist individuals and their families in coordinating, selecting and overseeing their own supports.
  • Informs providers and the Division about identified service gaps.
  • Utilizes available specialized and generic community resources in combination with other formal and informal supports and services.
Position Requirements
  • Bachelor’s Degree in a relevant discipline and 1-year equivalent experience.
  • Computer Proficient (MS Office Outlook, Word, Excel, PowerPoint).
  • Able to solve problems independently, effectively and creatively.
  • Excellent Organization and Communication skills.
  • Excellent time management skills to be able to juggle multiple plans for multiple families simultaneously.
  • Balance of work in the field and office- Must have a flexible schedule to meet the needs of our consumers.
  • Validate Driver’s license and car to be able travel throughout the state of New Jersey
