Case Manager - Intensive Community Support 


What are we looking for in a Case Manager?

Intensive Community Support Case Managers offer support to individuals who have a diagnosed mental health or substance use disorder and who are utilizing local emergency services or experiencing incarceration at a high rate. The goal of this position is to connect high-risk individuals with community-based treatment that enables clients to be proactive with their illness management and recovery, thereby reducing interactions with law enforcement and emergency services. To be successful in this role, Case Managers must enjoy variety in their day, possess exceptional communication skills, and be able to solve problems creatively. They must also be comfortable working in both emergency room and jail based settings.

Case Manager duties may include:

  • providing intensive support and case management services to clients, which includes transporting clients to appointments and assisting clients with accessing services
  • coordinating with clients' treatment teams and community providers to develop treatment and discharge plans, including planning for treatment upon release from correctional or medical facilities
  • monitoring symptoms & medication response and promoting illness self-management skills
  • using QPRT principles to assess clients for safety and provide appropriate interventions when indicated
  • completing all required progress notes in a client-centered, timely manner
Position Requirements

Education & Experience: Bachelor's degree in social work, psychology, or related field. Bilingual in English & Spanish required.

Professional Credentials: Eligible for Agency Affiliated Counselor registration

Other Requirements: Driver's license & vehicle liability insurance
