Case Manager- Wyandot Academy - Youth

Wyandot BHN is seeking a dedicated Case Manager to join our Wyandot Academy case management team! If you enjoy giving back to the community and working with youth in a collaborative team environment then this may be the job for you! The Case Manager - Wyandot, Academy affects improvements in the personal lives of youth seeking care by providing professional case management services.

Essential Job Duties:

  • Provide therapeutic support to PACES clients both in and out of the classroom at the Wyandot Academy program.
  • Ability to build and maintain positive working relationships with clients, parents, school staff, and other professionals.
  • Arrive in mornings to assist, triage, and meet needs of clients as they transition from home to school.
  • Work closely with clients and school staff to ensure specific client needs are being met in the school setting.
  • Develop lesson plans and facilitate evidence-based psychosocial curriculum for clients in daily psychosocial groups.
  • Participate in weekly team meetings to staff client successes, needs, and develop individualized treatment interventions.
  • Correspond with parents, case managers and other treatment team members to insure continuity of care.
  • Transport clients to and from home, as well as the community, during times worked in the PACES psychosocial program.
  • Coordinate with community based case managers and other staff to assist clients in accessing crisis services as needed.
  • Collaborate with all treatment team members in Wraparound services for clients.
  • Complete all documentation as trained within the company time limits.
  • Collaborate with schools, child welfare and other professional agencies.
  • Work as a group leader in PACES psychosocial programming when school is not in session--including Winter, Spring and Summer breaks.
 
Position Requirements

Job Requirements:

  • Bachelor’s degree in the human services field, with emphasis preferably in social work, psychology, rehabilitation or related field.
  • Valid driver’s license with motor vehicle insurance.
  • Must possess the minimum qualifications as documented in the Kansas Case Management Program for Children and Adolescents.
  • Familiarity with mental health theory, emerging promising practices, and Evidence-Based Practice.
  • Prior experience providing effective, efficient case management support preferred.
  • Demonstrated interpersonal and communication skills
  • Demonstrated knowledge of various job responsibilities, philosophies and values of PACES, including wraparound.
  • Ability to initiate and develop solutions in clinical, personnel, and other job related situations.
  • Ability to set clear expectations, provide appropriate feedback in work performance matters, and provide accountability for staff.
 
Full-Time/Part-Time Full-Time  
Shift Days  
Hours Monday-Friday, 8:00am-4:00 pm with occasional evening work.  
Pay Range $33-35k
