Therapeutic Support Specialist

The REACH Therapeutic Support Specialist provides support to persons with co-occurring developmental disabilities and mental or behavioral health needs who are temporarily accessing crisis or planned therapeutic stays within the REACH Crisis Therapeutic Home (CTH) In addition, this position functions as direct support to the CTH Supervisor providing administrative oversight for assigned program operations in the absence of the CTH Supervisor.

Essential Job Functions:

  • Oversees the therapeutic programming of the CTH including the development and implementation of therapeutic groups.
  • Leads therapeutic groups regularly to provide modeling/training for DSPs. Records and reviews video of group services weekly to provide feedback and strengthen staffs' skills.
  • Collaborates closely with the Clinical Director on all clinical aspects of CTH daily operations. Follows through with specific tasks and guidance provided by Clinical Director.
  • Establishes rapport with individuals with intellectual disabilities and challenging behaviors, while maintaining respect for each individual's dignity, individuality and right to confidentiality.
  • Maintains professional boundaries with individuals and follows ethical and licensing guidelines.
  • Monitors individuals for any mental health or behavioral symptoms and provides crisis prevention, intervention and de-escalation supports in accordance with crisis guidelines and the person's plan.
  • Serves as a positive representative for the agency with other professionals, families and the community.
  • Maintains timely documentation, progress notes, and other data, including incident and accident reports.
  • Responds to individual safety needs, including reporting significant changes in behavior and health to leadership and the medical team.
  • Adheres to program standards and policies as well as local, state and federal regulations.
  • Writes discharge summaries for guests upon discharge from the CTH.
  • Provides after-hours support/response to questions/needs from DSP team.
  • Functions as lead for on-site training of all new REACH DSP staff.
  • Participates in development and implementation of guests' Crisis Education and Prevention Plan, and ensures all staff are trained on each guest's CEPP.
  • Assists individuals with daily activities, personal care, behavioral health needs, and health, wellness and leisure activities as a part of training new staff, when staff shortages occur and/or when developing a support plan for a guest.
  • Assists in data collection related to mood, mental health symptoms, behavior, participation in therapeutic groups and activities, etc.
  • Identifies and incorporates principles of PERMA, positive psychology, and strengths-based perspectives into therapeutic support of CTH guests.
  • Identifies, administers, assigns, and trains staff on the implementation of appropriate therapeutic support tools, interventions, activities, and materials for every CTH guest in accordance with the guest's individualized goals and service outcomes.
  • Assists CTH Supervisor in ongoing assessment of program processes and operations and recommends and facilitates development of quality improvement measures with approval from the CTH Supervisor.
  • Assists CTH Supervisor in reviewing and processing all clinical documentation. Assures timely completion and accuracy of all clinical documentation by DSP staff.
  • Communicates with the REACH clinical team regarding CTH guests' overall presentation, behavior, mental health symptoms, strengths, vulnerabilities, triggers, effective intervention, etc.
  • Responds to CTH staff scheduling changes and emergencies. Contacts staff and authorizes scheduling changes (excluding PTO or overtime approval) as needed to best support program needs.
  • Ensures DSP staff are current on all required certifications/trainings. Assists CTH Supervisor in arranging training sessions as needed for initial and re-certifications of program staff.
  • Collaborates with CTH Supervisor in screening and interviewing DSP applicants.
Position Requirements
  • Graduation from an accredited college/university with a bachelor’s degree in counseling, social work, psychology, or Human Services related field, required..
  • At least 2 years’ experience working with individuals with IDD and behavioral health needs.
  • Qualified Mental Health Professional (QMHP) and/or Qualified Intellectual Disability Professional (QIDP) certification from the state board, required.
  • Knowledge of MH and I/DD systems of care; specific to Virginia preferred.
  • Capacity to effectively communicate verbally and in written form.
  • Knowledge and ability to utilize computer word processing and data programs including but not limited to Microsoft Office, Web-based health records.
  • Must possess valid, unrestricted, state appropriate driver’s license.
  • Use of reliable, personal transportation for work use may be required.
