Share Email Opening Apply Now Title Speech Language Pathologist (PRN)  Category Therapy   Position Requirements Position: PRN Speech Language Pathologist Reports to: Clinical Manager or Administrator Position Summary: Speech Pathologist's are responsible for providing therapy to patients at home or in alternative settings. Job Function:   Performs an initial assessment and evaluation of patient’s speech and language and reports evaluation findings to the physician and case manager promptly, and assists in establishing a plan of care. Plan for coordinated service for patients by consulting and working with other health care team members effectively and communicating schedules as assigned. Provides speech therapy services in accordance with applicable State, Federal, Medicare, Medical and other regulations and standards. Provides individual and group staff education programs, to prepare nurses and other discipline to identify and assist patients with their speech, language, swallowing, and communication problems, as necessary. Counsels and teaches patients and their caregivers regarding patients’ speech and language disturbances and provides support and educational materials to promote progress. Provides direct and indirect therapy services to patients based on a modified primary therapy approach. Such provision of care includes, but is not limited to: Patient assessment Patient and caregiver teaching Coordination of related services Development and revision of a therapy plan of care Implementation of physician orders Monitoring of patient response to care provided Completes all required patient care documentation and reports with accuracy and in a timely manner in accordance to internal and external regulatory requirements. Communicates with all disciplines on providing care for the patient including nursing staff, other therapy providers and physicians as appropriate to ensure quality and continuity of care. Guarantees use of proper protection during required work involving potentially hazardous agents and/or environments (i.e. handling of cytotoxic agents, blood and body fluids). Organizes daily work activities in accordance with patient needs. Requirements: Work experience of at least one (1) year under Speech Language Pathologist. Understands basic home healthcare regulations related to Health Insurance Portability and Accountability Act (HIPAA). Exercises good communication skills in working with the patient, family, field staff, office personnel, and referral sources. Manages a flexible work schedule (including weekends) in accordance with company attendance and reporting policies. Meets the education and experience requirement for a Certificate of Clinical Competence in speech pathology or audiology granted by the American Speech and Hearing Association, and valid licensed as a speech and Language Pathologist in the state of practice. Maintains access to reliable vehicle and has a willingness to travel Current CPR certification required Valid state driver’s license and automobile insurance coverage Home health experience is preferred.   Exempt/Non-Exempt Non-Exempt   EOE Statement We are an equal employment opportunity employer.
