Community Integration Counselor Lead 

Description

Individual Related Duties:

  1. Supervise and assist individuals in completion of daily (M-TH) written schedule of activities and routine such as personal care, meal assistance, transfer, lifting, etc., ensuring full participation of programming, as per individual needs and according to their Service Plan (SP) and Individual Service and Support Plans (ISSPs), completing documentation as per agency guidelines.
  2. Implement and document ISSPs, safety plans, and behavior management plans as stated; documenting after completion using progress notes and data collection forms specified per individual.
  3. Maintain required paperwork, including T-Log notes, ISSP data, billing/attendance in Therap, vehicle attendance, mileage logs, incident reports, etc., as per agency guidelines.
  4. Be aware of individuals with behavioral issues, providing appropriate intervention, counseling, and follow-up for unusual behaviors or reactions as needed. Document behaviors/reactions in case file and using behavior tracking and/or incident report form as per agency guideline.
  5. Assist with individual's SP meeting, 6-month review, IDT's, development of ISSP's, and Day Program plans as assigned. (Attend meetings if requested by supervisor).
  6. Assist in assuring that community and in-house activities are running in a timely and appropriate manner when PM is not available. Report any concerns to PM.
  7. Review required paperwork, including individual ID notes, vehicle attendance, mileage logs, incident reports, etc., as per agency guidelines. Be aware of individual behavioral issues, providing appropriate intervention, counseling, and follow-up for unusual behaviors or reactions as needed. Document behaviors/reactions using behavior tracking and/or incident report form as per agency guideline.

Agency Related Duties:

  1. Record actual hours worked daily on specified timesheet as per agency guidelines, submitting for supervisor's approval on last day of pay period or as otherwise directed. All leave requests must be submitted and approved by supervisor prior to leave, as per agency guidelines.
  2. Record all mileage using specified forms as per agency guidelines and submitting with mileage reimbursement form on last day of pay period for approval.
  3. Attend staff meetings and scheduled in-service as requested by supervisor and meeting agency in-service requirements, completing all assigned tasks within designated time frames.

Facility Related Duties:

  1. One-month prior to individual's SP meeting, ensure that annual assessment packet have been sent to CCBs by the assigned staff.
  2. Assist PM with preparation of SP documented prior to SP meeting and assist PM with any follow-up from SP meeting. Assist PM with the completion of Day Program master file monitoring's, as needed and facility monitoring.
  3. Monitor the day program facility on an ongoing basis (as needed), completing an environmental form. Completing community safety sheet for all new outing locations. Ensure the Day Program area is maintained in a clean and healthy manner, meeting guidelines on facility checklist and HUD on ongoing basis. Assist PM with plan of correction in deficient areas.
  4. Ensure completion and documentation of monthly safety drills per schedule, submitting documentation to PM.
  5. Maintain a Day Program book of inspections, maintenance and safety records as per agency guidelines.
  6. Complete and distribute summaries of individual progress (as assigned), end of month summaries for ISSPs. Review results with PM.
  7. Weekly review of attendance by the last day of each week and request follow-up from Day program staff, prior to turning in to billing.
  8. Complete programmatic monitoring on an ongoing basis (monthly), completing the proper form and distributing errors found to appropriate staff.
  9. Ensure completion and documentation of monthly safety drills per schedule, submitting documentation to PM.
  10. Maintain petty cash funds, making note of all transactions and ensuring accurate balance as verified by PM, dispersing as per set guidelines, and requesting additional funds as needed. Working with staff who is assigned for calendar and reviewing budget and sending to PM for approval prior to the calendar distribution.
  11. Ensure client to staff ratio is being met and assist in coordinating and finding coverage when PM is not available.
  12. Ensure completion and documentation of monthly safety drills per schedule, submitting documentation to PM.
  13. Assist staff assigned to the monthly calendar to prepare monthly list of materials needed for daily activities and submit to PM. Once approved, purchase all the materials prior to the event.
  14. Maintain petty cash funds, making note of all transactions and ensuring accurate balance as verified by PM, dispersing as per set guidelines, and requesting additional funds as needed.

Expectations for Completion of Job Duties:

  1. Display professional appearance and behavior, interacting with other staff, individuals, parents and service providers in a cooperative and friendly manner as observed by supervisor.
  2. Always provide positive role model at Day Program and in the community, avoiding use of profanity or derogatory slang, arriving to work in proper attire, modeling good problem-solving skills and social interactions.
  3. Perform job duties with positive attitude, responding to peak work period without loss of quality, accuracy and thoroughness.
  4. Uphold individual rights by following individual rights guidelines and agency abuse, neglect, and mistreatment policy daily.
  5. Punctual and dependable at reporting to work (i.e. absenteeism and lateness does not result in adverse affects on operation of programs, individuals or families served).
  6. Attend regular and as-scheduled payday meetings, management meetings, in-service training classes, conferences, and other meeting as assigned.

 
Position Requirements

GENERAL REQUIREMENTS: Degree or training in related field. Must have good organizational skills, excellent writing and planning skills and the ability to work independently given general job expectations. Previous experience with CP or CIE for this population is required. Must have valid driver’s license, reliable transportation, and proof of insurance.

Supervision Received: Works directly under supervision of the Community Services Manager, Program Director or President/CEO available to resolve unusual work problems.

Hours worked: 36 to 40-hour work week with flexibility based on individual and program needs.
