We are seeking mission focused professionals to serve as a full time Case Coordinator for the state of South Carolina who will be responsible for providing case management and in-home support service to Caregivers and Consumers in our Community Training Home Program. The Community Training Home Program serves adults with a physical, mental, or emotional disability living in the community under the care of a licensed Caregiver. This program is Sponsored by the Department of Disabilities and Special Needs.

As a Case Coordinator you would:

  • Complete annual assessments and residential support plans.
  • Provide monthly visits to homes located across the state. Travel time is approximately 25-30%.
  • Serve as the Representative Payee for Consumers. Ensuring financial management of consumer funds. Ensure timely submission of financial documents.
  • Provide case management and coordinate Behavior Support Services, Day Programs, and assist with Job Placements as needed.
  • Quarterly train Caregivers and Consumers on Consumer Rights, Abuse, Neglect, and Exploitation.
  • Participate in On call rotation for Consumers.
  • Create documentation for all services you provide in our Electronic Record System. Documentation includes Assessments, Residential Support Plans, and Progress Notes, and Monthly reports. Overall documentation time is about 40%.
  • Provide in-home, direct services to consumers and caregivers on your caseload. Generally, caseloads consist of 11-12 consumers with whom you visit 1 time per month.
  • Develop and maintain consultative and liaison relationships with DDSN and other members of the consumers treatment team (Behavior support person etc.)
  • Work standard hours of around 8am - 5pm initially. The schedule will flex as you receive your caseload and begin seeing families based on their availability. This includes potentially working until 8pm several nights a month, based on the needs of the families.
  • Provide required training to active caregivers and support recruitment efforts in the community; occasional Saturday hours occur with weekday time flexed to support work/life balance.
Position Requirements

What you bring to this role:

  • Bachelor’s degree in social work, counseling, psychology, sociology, or child & family studies is required. Master’s degree preferred.
  • 2+ years of experience working with families and children, individuals with disabilities, child welfare, in-home therapies, DSS.
  • Passion for serving, knowledge in trauma focused car; time management, organization and effective communication skills; a desire to work as a part of a dynamic team.
